If you are talking about the Home Instructor's Guides then yes I have them for SM 2a and up. They are very helpful for correcting work and when I need some extra help explaining the problems. I would definitely get them for those grade levels. :)

I would go ahead and just finish out LHFHG. You really don't want to rush them through the guides and get to the upper ones too soon. I would start the ER's if she is ready and also continue with Phonics. That is what I do with my dc. They are usually ready for the ER's before we are done with phonics. :)

All four of my kids are in separate guides. The closest age span I have is 22 months but even they are separate. So far that has been easier for me. I tried to combine my older two at first but it didn't work for them.

Yes the entire texts are used. Personally I think it would be overkill to add in the Apologia Notebooks. HOD already has the kids doing notebooking along with their science readings. My ds is doing CTC right now and his school days are very full. I can't imagine asking him to do additional writing. ...

We do Spanish right after lunch and recess between seat work and read alouds. I do not do a worksheet program and we don't spend a ton of time. I have not read ahead for my ds. I thought about doing that this summer but didn't get to it. We started CTC last week. My ds loves the reading part of CTC so it isn't really an issue for him and I can tell by his work what he is learning and if he understood it.

LHTH doesn't take long at all but it is really fun. It is scheduled to have 5 days per unit but you don't have to do it 5 days a week. Yes you would move into level 3 if you completed 2 in Bigger. Level 4 is in there for people using Preparing in the extension range or if a kid is advanced and needs to move ahead in dictation. The same with level 2 being in there for kids who weren't ready for dictation with Bigger.

Nope, just keep working at it one day at a time. If you don't finish a unit in a week just start again where you left off on the next school day. That is what we do anyways. Poor guy. :( My older dd got a concussion from falling backwards off of the back of our coach in the spring. Hers was milder than your sons it sounds like but she was back to normal after a few days. Kids are amazing like that sometimes. I hope your son recovers quickly.
